How do I format a ban appeal?

I got banned by the owner. It said
You have been banned for Appeal at Snapcraft.net by iMaartennn permanently
I did not understand the reason I was banned from the message which means I can't prove myself innocent. I was going to appeal but I was so confused about why I got banned in the first place. Not to gain sympathy, but I had all my stuff on that account, and I would appreciate if someone showed me the format of an appeal.

Open a support ticket here. Change department to "Appeals". Fill in the requirements, talk about why you think you should be unbanned (+ additional stuff, that's optional).
